Dermatologist’s Tips to Revive a Severely Dry Scalp
Dry scalp is usually caused by a rash. Skin appears dry because it is flaky. But, the flakes are due to a rash, not dryness!
Did you know that scalp skin is actually rich in oil glands and can’t suffer from dry skin?
Those same oil glands put the scalp skin at risk for seborrhea (dandruff), the most common cause of flaky and dry-appearing scalp. Treat the rash and get rid of the flakes!
Seborrhea is most easily-controlled using a medicated dandruff shampoo and a good scalp scrubber. Options include Foaming Zinc Cleanser, and my favorite scalp scrubber.
When this dynamic duo doesn’t work, it’s time to move to more intensive treatments like deep oil treatments and leave-on medicated products:
Intensive Scalp Treatments to Remove Thick Scale and Control Stubborn Dandruff
Warm Oil Scalp Treatment
If you have stubborn plaques of really thick scale, you can loosen them with oil treatments or P&S liquid.
Oil Treatment
Mineral oil or coconut oil can be warmed gently in the microwave. Of course, you need to be careful not to heat it to the point that it burns your skin!
My oil preference for scalp treatment is extra virgin coconut oil because it has natural antimicrobial properties. It also seems more effective and has the added benefit of making your hair shiny and soft. Simply:
Apply the warmed oil to the scalp skin for about 30 minutes under a towel; Comb the scalp to help loosen the scale; Then, shampoo your scalp to remove the oil; Use your medicated shampoo as your last lather; Scrub it into the skin with the scalp scrubber; Allow it to sit on your scalp for a few minutes, and then rinse; and You can use conditioner or your normal styling products after using a medicated shampoo.P&S Liquid Scalp Treatment
P&S Liquid is a product with medicine that loosens scale. Apply the liquid to the stubborn, scaly areas of your scalp and cover with a shower cap for 8 hours. Then, shampoo your scalp as described above.
Regardless of which of the two intensive-treatment-methods you use, you will need to repeat them until the dryness and flaking is controlled.

Topical Leave-on Medicated Products to Boost Control of ‘Dry Scalp’ from Dandruff
For stubborn dandruff, and to speed relief when you are using medicated shampoos, you can apply leave on-products to the affected scalp skin.
Medicated options available without prescription include hydrocortisone and salicylic acid liquids. Usually, the product will instruct you to apply the medicine twice a day to your scalp skin until the dryness clears. Remember to follow the directions on the product labels.
Product options include:
Scalpicin Maximum Strength with 1% Hydrocortisone; and Scalpicin with 3% Salicylic AcidStronger medicines are available from your doctor for really stubborn scalp dandruff.
Once the dryness is under control, here are tips to help keep your scalp looking great:
When the severe flakes are gone, keep them from returning with the daily use of a medicated dandruff shampoo. Scalp skin sometimes quits responding to a single ingredient, and you may need to switch things up.
Dandruff shampoos are made from the following ingredients:
Pyrithione zinc (2% is the highest concentration and the one I recommend); Salicylic Acid with or without sulfur (2-3% salicylic acid is best); Ketoconazole; Selenium Sulfide; and TarLook for different, active ingredients, and either rotate them or alternate them by using a different ingredient each time you wash your hair. I often tell my patients to have three different ingredients and use a different one each day.
My favorites include zinc pyrithione, salicylic acid and ketoconazole. This provides treatment with three different mechanisms of action.
Can you treat your scalp like your beard to help keep skin free of “druff”?
Yes! The scalp and the beard-area are both prone to scale build-up from dandruff. If your scalp-hair is short, you can use a beard oil to help condition your skin much like you do for your beard.
Use a beard oil, and apply it to the skin with your fingertips, rubbing it under the hairs so that the skin is treated. As the oil warms to your skin temperature, it will spread to cover your skin.
Use a beard oil that is gentle on the skin but has botanical ingredients known to fight inflammation that plays a role in dandruff.
My top choice is my Omega Enriched Booster Oil – a great beard-oil enriched with organic Sea Buckthorn Oil, Borage Oil, and Cypress Oil that also imparts a subtle and natural woodland scent.
Please remember that if your scalp does not improve, you should see your doctor to be certain that you don’t have some other, more unusual, reason for an itchy and scaly scalp.
There are a number of conditions that can cause scalp scale including psoriasis and ring worm (fungus) of the scalp. These two common conditions can mimic really stubborn scalp dandruff.
